Biceps brachii long tendon dislocation.

Abstract

Background: Dislocation of the biceps tendon at the level of the biceps slide associated with anterior gleno-humeral dislocation is uncommun, most of the times undiagnosed. Clinical case: 54-year-old male, farmer, who suffered dislocation of the shoulder by horse fall, he had a closed reduction and immobilization. Presenting anterior pain in right shoulder. On physical: deltoid hypotrophy, mobility arcs limited to: 80 ° flexion, 40 ° extension, 45 ° abduction, 20 ° external rotation and 10 ° internal rotation, with positive results for  tests like: Neer, Jobe, Apley, Belly, Yergason, Speed ​​and forearm supination.  With diagnosis of  long tendon biceps dislocation, magnetic resonance was requested, corroborating: a) extrarticular dislocation of the tendon of the long bicipital portion at the level of its humeral slide due to total rupture of the ligament transverse humeral and subscapular tendon.

Conclusions: Shoulder dislocations are frequent, its association with dislocation of the extrarticular biceps tendon at the level of the bicipital slider is rare, implying extension of the subscapular tendon rupture and transverse ligament, which may not noticed.
